Walk into any clinic, emergency room or nursing home and the same four measurements happen before almost anything else: temperature, pulse, respiratory rate and blood pressure. They are quick, inexpensive and non-invasive, and together they give a remarkably good first picture of how a body is functioning. Understanding what each one measures — and what makes it change — is foundational knowledge in nursing and in first aid alike.
Why these four?
Vital signs earned their name because they reflect the functions the body cannot go without: maintaining a stable internal temperature, circulating blood, exchanging gases, and keeping enough pressure in the vessels to deliver oxygen to tissue. When one of these drifts outside its usual range, something upstream is usually driving it.
Two additional measurements are so widely used today that many textbooks now include them: oxygen saturation, measured with a fingertip pulse oximeter, and pain, rated on a subjective scale. But the classic four remain the core.
Body temperature
Humans maintain a narrow internal temperature because enzymes — the proteins that run our chemistry — work properly only within a limited range. The hypothalamus acts as the thermostat, triggering sweating and blood vessel dilation to lose heat, or shivering and vessel constriction to conserve it.
Temperature is not a fixed number. It varies with the time of day (typically lowest in the early morning), with physical activity, with hormonal cycles, and with the site of measurement. Rectal and ear readings tend to run higher than oral readings, and armpit readings tend to run lower.
A fever is not the illness itself — it is the body deliberately raising its own set point, usually as part of an immune response. That distinction matters clinically: the useful question is what caused the fever, not simply how to lower the number.
Pulse
The pulse is the wave of pressure travelling along the arteries each time the left ventricle contracts. Counting it tells you the heart rate, but a careful assessment records three things:
- Rate: beats per minute.
- Rhythm: regular or irregular, and if irregular, whether the irregularity itself follows a pattern.
- Strength: how forceful the beat feels — described as bounding, normal, weak or thready.
The radial artery at the wrist is the usual site in a conscious adult. The carotid artery in the neck is used when circulation is poor or in emergencies, and the brachial artery is standard for infants.
Resting heart rate falls with cardiovascular fitness — trained endurance athletes often sit well below the usual adult range without anything being wrong. It rises with fever, dehydration, blood loss, pain, anxiety, caffeine and many medications.
Respiratory rate
Respiration is the one vital sign a person can change consciously, which creates a practical problem: telling someone you are counting their breaths tends to alter the result. The standard technique is to count breaths immediately after taking the pulse, while still holding the wrist, so the patient does not realise the focus has shifted.
One breath means one full cycle of inhalation and exhalation. Alongside the rate, note the depth (shallow or deep), the effort (is the person using neck and shoulder muscles?) and any sound (wheezing, gurgling, stridor).
Respiratory rate is often described as the most sensitive early warning sign of deterioration, and also the one most frequently rushed or estimated rather than properly counted.
Blood pressure
Blood pressure is reported as two numbers separated by a slash:
- Systolic (the upper number) is the peak pressure in the arteries while the heart contracts.
- Diastolic (the lower number) is the pressure remaining while the heart relaxes and refills.
Both depend on how much blood the heart pumps and how much resistance the vessels offer. Narrowed or stiffened arteries raise the pressure; blood loss or widespread vessel dilation lowers it.
Technique matters enormously here, and small errors produce large deviations:
- The cuff must fit the arm. A cuff that is too small gives falsely high readings; too large gives falsely low ones.
- The arm should be supported at roughly heart level. An arm hanging down reads higher.
- The person should be seated, back supported, feet flat, and resting quietly for a few minutes beforehand.
- Talking during the measurement raises the reading.
- Legs crossed at the knee also raises it.
Because a single reading can be misleading — the well-documented “white coat” effect being one reason — diagnosis of hypertension relies on repeated measurements on separate occasions, not on one office visit.

These are general reference ranges for healthy adults, and they vary between guidelines and populations. Children have markedly different normal values — infants breathe and beat far faster than adults — and people with chronic conditions may have a personal baseline that sits outside the textbook range. Interpretation always belongs to a qualified professional.
Trends matter more than single numbers
A single reading is a snapshot; a series of readings is a story. A pulse of 96 means little on its own, but a pulse that climbed from 68 to 96 over three hours while blood pressure drifted downward means something quite different. This is why observation charts exist and why documenting the measurement — with the time, the position of the patient and the site used — is as important as taking it.
The same logic applies to home monitoring. Someone tracking their own blood pressure gains far more from a consistent log taken under consistent conditions than from occasional readings taken whenever they happen to feel unwell.
Common measurement errors
- Counting the pulse for 15 seconds and multiplying by four when the rhythm is irregular — irregular rhythms need a full minute.
- Measuring immediately after the person has walked in, climbed stairs, smoked or had coffee.
- Placing a blood pressure cuff over clothing.
- Using a pulse oximeter on a cold finger, or one with nail polish, which can distort the reading.
- Recording numbers without noting the context in which they were taken.
