- Feature magnetism: Shiny, concrete deliverables crowd out less visible work (maintenance, research, documentation).
- Pet-project gravity: Ideas with strong internal sponsors get disproportionate airtime.
- Ambiguous criteria: When “impact” isn’t defined, teams substitute what feels persuasive: vivid anecdotes, confident presenters, or the latest customer complaint.
Retrospectives and incident reviews
- Outcome-based judgment: Teams judge decisions by results rather than by what was knowable at the time, which discourages smart risk-taking and learning.
- Single-cause stories: Complex events get simplified into one “root cause,” often a person or a single mistake, instead of a system of contributing factors.
- Participation imbalance: A few voices dominate, and the team mistakes loudness for representativeness.
Performance reviews and promotions
- Uneven evidence: Managers rely on memory and a few salient moments instead of a balanced record across the review period.
- Role ambiguity: Without explicit expectations, reviewers reward style, similarity, or visibility rather than outcomes and behaviors tied to the role.
- Comparability problems: People are judged against different standards depending on project difficulty, stakeholder proximity, or who advocates for them.
Strategy decisions
- Thin data, thick confidence: Strategy often uses incomplete information; teams can mistake a coherent narrative for a validated plan.
- Consensus pressure: Leaders may unintentionally signal the “right answer,” narrowing exploration and reducing dissent.
- Commitment lock-in: Once a direction is announced, it becomes socially costly to revisit assumptions.
2) Structured Decision Artifacts (Make Judgment Auditable)
Structured artifacts turn fuzzy discussions into comparable evidence. They reduce noise, prevent decisions from being dominated by charisma or timing, and create a trail you can learn from later.
A. Scorecards (for hiring, vendor selection, promotions, project selection)
Purpose: Define what “good” looks like before you meet the candidate, see the demo, or hear the pitch.
How to implement (step-by-step):
- Step 1: Define criteria tied to the role/outcome. Example for a product manager: problem framing, stakeholder alignment, experiment design, written communication.
- Step 2: Create behavioral anchors for each criterion. Use 1–5 scales with descriptions (e.g., “3 = can run a basic experiment with guidance; 5 = designs robust tests, anticipates confounds, and communicates tradeoffs”).
- Step 3: Require evidence notes. Every score must cite specific evidence (quote, work sample, metric, observed behavior).
- Step 4: Score independently first. Reviewers submit scores before discussion to prevent social influence.
- Step 5: Debrief by criterion, not by person. Compare evidence for “communication” across interviewers before discussing overall fit.

- Context: Why now? What changed?
- Options: 2–4 viable alternatives (including “do nothing”).
- Tradeoffs: Costs, risks, dependencies, reversibility.
- Recommendation: Proposed choice and rationale.
- Open questions: What would change your mind?
Process rule: If the pre-read is not sent in advance, the meeting becomes a working session (no decision) or is rescheduled.
C. Decision logs (for strategy, prioritization, incident follow-ups)
Purpose: Capture what you decided, why, and what you expected—so learning is possible later.

Devil’s advocate: Challenges the strongest argument, not the weakest person. Rotates each meeting.
Red team: A small group tasked with stress-testing a plan as if they wanted it to fail. They produce a short critique: failure modes, missing data, and counterexamples.
Facilitator: Protects process: agenda, timeboxes, equal participation, and decision clarity.
Scribe: Captures decisions, assumptions, and action items in real time.
Decision owner: Accountable for the call and for documenting it; not necessarily the highest-ranking person.
3) Separate Generation from Evaluation (Stop Early Convergence)
Many team failures come from mixing two incompatible modes: generating options (creative, expansive) and evaluating options (critical, selective). When evaluation starts too early, the first plausible idea becomes the default, dissent feels like conflict, and teams converge before exploring.
A simple facilitation pattern
- Phase 1 — Generate (diverge): Create many options without judging them.
- Phase 2 — Clarify: Ask questions to understand each option; still no ranking.
- Phase 3 — Evaluate (converge): Apply agreed criteria; compare tradeoffs.
- Phase 4 — Decide: Choose, assign owner, define success signals and revisit triggers.
Techniques that enforce separation
- Silent writing first (5–10 minutes): Everyone writes ideas independently before discussion. This reduces “follow-the-first-speaker” dynamics and increases idea diversity.
- Round-robin share: Each person shares one idea at a time until exhausted; prevents domination.
- Two-column board: Options vs Evaluation. Ideas go left; critiques go right only after the generation timer ends.
- Criteria lock: Agree on evaluation criteria before scoring options (e.g., impact, effort, risk, reversibility, time-to-value).
Example: Prioritization without early convergence
Scenario: The team must choose one of five initiatives for the next sprint.
- Generate: Each person proposes 1–2 initiatives plus a “do nothing / stabilize” option.
- Clarify: For each initiative, capture: user problem, expected impact metric, dependencies.
- Evaluate: Score each initiative 1–5 on impact, confidence, effort, and risk. Require a one-sentence evidence note per score.
- Decide: Pick top candidate; record assumptions and a revisit trigger in the decision log.
